I really like the background music and the atmosphere sound! It reminds me of Animal Crossing a bit... The graphics look interesting too.
It did take me a while to understand the bars on islands (well, you have to see an island sink before you know what it means :p)
Very cute, fun, good looking game! Great job on your first Ludum Dare (and hey it's my first LD too and I also enjoyed it a lot!)
One suggestion would be to simplify the control a bit. I saw that I can destroy the virus by either clicking on them directly or by dragging the antibody to collide with them. The former was a bit confusing to me. If you remove the clicking control, you can just make the antibody always follow the cursor!
Also! Great use of Unity post processing and great sound design!
The background sound effects really produces a great atmosphere for this game.
I like that the screen is tinted when one of the vital signs goes off. It's a great indicator of how I'm performing and provide instant & intuitive feedback.

A little feedback on sound: the sound of heart beating clashes with the music (drums), making it a bit unpleasant
